Our vision is to create health equity for all our community members. We want to ensure every voice is heard and every need is addressed. Using innovative, community-designed initiatives, the Health & Wellness team are able to address unmet needs of the community including access to vaccines, insurance, housing, and overall health needs.

  • Provided hundreds of boxes of PPE and At-home Covid tests to local families.  
  • Helped over 200 community members get vaccinated against Covid-19  
  • Provided ongoing basic needs for hundreds of local children  ​

LWK Change Model ​

  • To achieve our health equity vision, we use SDoH framework knowledge for guidance. 
  • We believe the underlying causes of health inequities include disparities in education, income, housing, built environment, economic stability, and social and community context and community connectedness. 
  • These social determinants of health, and addressing them in cultural context is necessary to achieve toward health equity. 
  • Creating health equity is our guiding principle. By health equity, we mean everyone has the opportunity to attain their highest level of health and well-being. ​
